PLLA-based dermal fillers, such as Sculptra® and Ecolla®, represent a new generation of injectable treatments that focus on restoring volume gradually and naturally by stimulating your skin’s own collagen production.
Unlike traditional hyaluronic acid (HA) fillers that provide immediate volume, PLLA (poly-L-lactic acid) fillers work deeper, with results that evolve and improve over time. PLLA Fillers don't just fill - they rebuild your skin's foundation.

How PLLA Filler Works
PLLA is a biocompatible, biodegradable synthetic polymer that has been safely used in medical devices for decades. When injected into the dermis or subdermal layer of the skin, PLLA acts as a collagen stimulator.
Once injected:
-
The PLLA microparticles begin a mild inflammatory response, activating fibroblasts.
-
This triggers neocollagenesis — your body’s natural process of generating new collagen.
-
Over the following weeks and months, the treated areas gradually thicken and become firmer as collagen builds.
-
The PLLA particles are broken down naturally and absorbed by the body, leaving behind newly formed collagen for long-term support and volume.
The result is a progressive, subtle, and natural-looking rejuvenation that restores skin structure and youthful contours from within.
What to Expect During & After Treatment
-
Consultation & Assessment: Every treatment begins with a detailed consultation to map out facial volume loss and determine suitability.
-
Injection Process: The product is typically reconstituted 24–48 hours before treatment and injected via cannula in targeted areas.
-
Mild Swelling: Swelling post-treatment is common and usually resolves within a few days. This initial swelling is not the final result.
-
Gradual Improvement: Results are not immediate. Collagen synthesis begins around week 3 and continues over several months.
-
Massage Protocol: Patients are advised to follow the “5-5-5 rule” — massage the treated areas for 5 minutes, 5 times a day, for 5 days to ensure even product distribution and optimum results.
Who Will Benefit From PLLA Filler?
PLLA fillers are ideal for:
-
Clients with significant volume loss, facial hollowness, or skin laxity.
-
Those wanting a gradual, natural-looking lift without the “overfilled” look.
-
Men and women aged 30+ who wish to restore youthful structure to cheeks, temples, jawline, or chin.
-
Individuals with age-related collagen depletion, thin skin, or deeper folds that cannot be effectively treated with HA fillers alone.
-
Clients looking for longer-lasting rejuvenation (results can last up to 2 years or more with proper treatment).
It is especially beneficial for those who are:
-
Post-weight loss or post-menopausal (where collagen loss is more pronounced)
-
Experiencing facial sagging or “deflation” without wanting surgery
How Many Treatments Are Required?
PLLA filler is not a one-and-done treatment. A course of treatments is typically recommended for optimal results:
-
Most Clients require 2–4 sessions, spaced 4–6 weeks apart.
-
Severe volume loss or more mature skin may require up to 4 sessions.
-
Full results usually appear over 3–6 months after the initial session.
Results build over time and can last 24+ months, making PLLA one of the longest-lasting non-surgical facial volumisers available.
Choosing Between PLLA Fillers and Traditional Dermal Fillers
When it comes to restoring volume and rejuvenating the face, there’s no one-size-fits-all solution. That’s why we offer both PLLA (Poly-L-Lactic Acid) fillers and traditional hyaluronic acid dermal fillers in Clinic — each with its own unique benefits, depending on your skin goals, lifestyle, and how gradual or instant you want your results to be.
PLLA fillers (such as Sculptra or Ecolla) work differently from traditional fillers. Rather than simply filling areas for immediate volume, PLLA stimulates your body’s own natural collagen production over time. This means results are gradual, long-lasting, and incredibly natural. You’ll begin to notice improvement in volume, texture, and skin firmness over several weeks, with full results typically seen after a course of 2-4 treatments, depending on individual factors such as skin texture, condition, desired outcome, and the degree of volume loss.. The effects can last up to two years or more, as your skin continues to regenerate and rebuild its internal structure.
On the other hand, traditional dermal fillers made from hyaluronic acid offer instant results. They are ideal for areas that need immediate definition, such as the lips, cheeks, jawline or under-eye area. These fillers can be moulded and shaped precisely, offering a more tailored approach to facial sculpting. The results are visible straight away and typically last around 6 to 12 months, depending on the area treated and your individual metabolism.
Clients who prefer a subtle, gradual transformation and are focused on long-term collagen support often opt for PLLA. It’s perfect for those noticing age-related volume loss, skin laxity, or who want to improve overall skin quality without looking like they’ve “had work done.” Traditional fillers are often chosen by those seeking targeted enhancements or quick results for events or photos.
Many clients even combine both treatments — using PLLA to rebuild collagen and maintain long-term skin health, alongside hyaluronic acid fillers for areas where sharper definition or instant lift is desired.
During your consultation, Mel will assess your skin, discuss your goals, and help guide you toward the best treatment (or combination of treatments) for your unique face and lifestyle.
Contraindications for PLLA Fillers
🔴 Absolute Contraindications
These are conditions under which PLLA fillers should never be administered.
Pregnancy and Breastfeeding
-
No safety data exists for PLLA use in pregnant or lactating individuals.
-
Elective aesthetic treatments should be deferred during these times.
-
Active Skin Infection or Inflammation at Injection Site
-
Bacterial (e.g., cellulitis), viral (e.g., herpes simplex), or fungal infections must be fully resolved prior to treatment.
Autoimmune or Connective Tissue Disorders
-
Conditions such as lupus, rheumatoid arthritis, scleroderma, or Sjögren’s syndrome may increase the risk of exaggerated inflammatory responses or poor healing.
-
Treatment may be considered on a case-by-case basis with physician clearance.
Known Hypersensitivity or Allergy to PLLA or Any Components of the Product
-
This includes allergy to carboxymethylcellulose (CMC) or mannitol, both commonly found in PLLA formulations.
Tendency to Form Hypertrophic Scars or Keloids
-
Collagen stimulation may exacerbate fibrotic scarring in predisposed individuals.
Active Cancer or Undergoing Chemotherapy or Radiotherapy
-
Immunocompromised status and poor healing ability are strong contraindications.
History of Severe Allergic Reactions or Anaphylaxis
-
Especially where there has been a history of adverse response to dermal fillers or injectable products.
Uncontrolled Diabetes or Other Conditions with Impaired Wound Healing
-
Poor tissue repair and increased risk of infection may complicate the procedure.
Recent Laser or Deep Skin Treatments
-
Skin must be fully healed before PLLA injection to reduce the risk of unwanted side effects or irregular collagen formation.
🟠 Relative Contraindications & Cautions
These conditions don’t absolutely rule out treatment but require extra care, discussion, or medical sign-off.
Autoimmune Conditions in Remission
-
May be considered in some cases with written consent and GP/consultant approval.
Blood-Thinning Medications
-
Including aspirin, NSAIDs, warfarin, or supplements like fish oil, vitamin E, and ginkgo biloba. These increase the risk of bruising and should be avoided 48-72 hours pre and post treatment where possible.
-
Clients may be advised (with GP guidance) to pause non-essential supplements.
Previous Dermal Fillers in the Area
-
HA fillers or other materials in the same area can interfere with PLLA distribution and outcomes. Careful assessment is required.
History of Nodules or Granulomas from Other Fillers
-
A history of inflammatory response increases the risk of a repeat occurrence with PLLA.
Recent Dental Work or Facial Surgery
-
Wait at least 2–4 weeks post dental or facial procedures to reduce infection risk and allow healing.
Unrealistic Expectations or Body Dysmorphic Disorder (BDD)
-
Clients must have clear, achievable expectations and understand that PLLA is a gradual treatment, not an instant solution.
Active Acne or Rosacea Flares in Treatment Area
-
Inflammation may increase the risk of complications or uneven collagen stimulation.
⚠️ Other Considerations Before Treatment
-
Poor Compliance – Clients unwilling or unable to follow the post-treatment massage routine (5-5-5 rule) may not be suitable, as this step is critical to avoid nodule formation.
-
Travel or Events – If the client has upcoming events or long-haul travel planned soon after treatment, they should consider the downtime from swelling and the delayed results.
-
Immunosuppressant Medication – May impact healing or alter immune response. Assess case-by-case.
✅ Client Safety Comes First
At our clinic, all clients undergo a thorough consultation, full medical history review, and skin assessment before PLLA filler treatment. If you’re unsure whether you are suitable, please don’t hesitate to book a consultation — your safety and satisfaction are our top priority.
PLLA Filler Aftercare Advice
Essential guidance to ensure optimal results and avoid complications
Poly-L-lactic acid (PLLA) fillers such as Sculptra® and Ecolla® are unique in that they stimulate your own collagen production over time, rather than providing immediate volume. Because of this, post-treatment care plays a critical role in achieving smooth, natural-looking, and long-lasting results.
Here’s everything you need to know following your treatment:
✅ Immediately After Treatment
-
Expect mild swelling or puffiness — this is temporary and due to the water content of the product and the body’s healing response.
-
Redness, bruising, or tenderness at injection sites are also common and should subside within a few days.
-
Do not be alarmed if your results seem to “disappear” within a few days — the visible volume is from swelling and fluid, not yet from collagen. The true results develop gradually over 6–12 weeks.
💆♀️ Massage – The 5-5-5 Rule
One of the most important aftercare steps with PLLA - Massage the treated areas:
-
5 minutes
-
5 times a day
-
For 5 days
Massage ensures even distribution of PLLA particles and reduces the risk of lump formation under the skin. Use a gentle, circular motion with clean hands or a moisturiser. Mel, your Practitioner will show you exactly where and how to massage.
🚫 Things to Avoid in the First 24–48 Hours
-
No makeup or skincare on treated areas for at least 24 hours to reduce infection risk.
-
Avoid alcohol, as it may increase bruising.
-
No strenuous exercise or heavy sweating for 24–48 hours, as this may worsen swelling and bruising.
-
Avoid saunas, steam rooms, hot baths or showers, and other sources of heat that can dilate blood vessels and increase swelling and bruising.
-
Do not rub or press the area, apart from the instructed massage.
🧊 Minimising Swelling & Bruising
-
Apply cool compresses (wrapped in cloth) for 10–15 minutes at a time to reduce swelling. Do not apply ice directly to the skin.
-
Sleep with your head slightly elevated the first night to reduce fluid retention.
-
If bruising occurs, you may use Arnica cream or oral Arnica supplements (if safe for you) to support healing. Clients who are currently taking blood thinning medication should avoid arnica, unless cleared by their GP. Arnica may enhance the effect of anticoagulants (e.g., warfarin, aspirin, clopidogrel) or anti platelet drugs, potentially increasing the risk of bleeding or bruising.
🕰️ When to Expect Results
-
Collagen production starts gradually and becomes more noticeable around week 3–4.
-
Optimal results are typically seen after the second or third treatment, and continue to improve over 3–6 months.
-
Most clients require 2–4 sessions, spaced 4–6 weeks apart.
📅 Follow-Up Treatments & Maintenance
-
Mel will recommend the best schedule based on your facial volume loss and collagen health.
-
A full course of treatments typically provides results lasting up to 2 years.
-
Maintenance treatments may be suggested annually to preserve collagen and facial structure.
🚨 When to Contact the Clinic
Call the clinic if you experience:
-
Significant or increasing swelling after 48 hours
-
Lumps that do not soften with massage
-
Signs of infection (heat, redness, pus, or fever)
-
Severe or prolonged pain
-
Blanching (white or pale skin) or discolouration
📝 Final Tips for Best Results
-
Stay hydrated and follow a healthy diet — collagen synthesis is supported by good nutrition, hydration, and healthy lifestyle habits.
-
Avoid smoking, which breaks down collagen and may reduce the longevity of your results.
-
Protect your skin from sun damage — use SPF 30+ daily, as UV exposure accelerates collagen breakdown.
